This Form is for use by members of St. John's Lodge No. 9 F&AM of Washington only. It is for the purpose of requesting Lodge support for a member's favorite charity.

St. John's charitable support is directed primarily toward local needs with low administrative costs. A supported charity should be apolitical and non-sectarian. Preference is given to a member's personal involvement. Our focus is on assistance to issues of health, education, youth and the aged.
